Download SURPASS Checklists
The SURPASS checklist is provided with copyright to indicate this is the original evidence-based checklist. Every hospital can use the SURPASS checklists on paper, provide it with their own logo, and if needed modify it(though this might have a negative effect on it's efficiency).
It is recommended to make use of the track-and-trace SURPASS Digital application instead of the paper version. This application allows checklist to be entered digitally from within most hospitals own patient related application (e.g. Patient Health Record applications).

It is not allowed to use the SURPASS checklist for commercial purposes.
